Changes to Patient Insurance or Status
It is always the patient's or legal caregiver's personal responsibility to inform all of their doctors of any changes to demographics, insurance, or hospice status.
This must be done as soon as the change is completed to ensure your providers can address the changes and reduce the risk of costly out of pocket expenses.
Authorizations and Referrals
Please note that if your insurance requires an authorization or referral, it is always the patient's responsibility to have seen their primary care provider and obtain one. Our office performs this service as a courtesy, but the responsibility and contractual obligation remains that of the patient.
If your appointment needs to be rescheduled because a valid authorization/referral is not on file, you may be charged a service fee. Please always verify prior to your appointment when we call you to confirm your appointment if you are unsure.

Out of Network and Not Accepted Insurance Plans
***We are out of network with certain plans or carriers and we reserve the right to drop any insurance or plan at any time for any reason.
Even though we may be out of network or do not accept your insurance, we may still see you out of network at our current self pay rate. Out of network costs are determined by the current annual medicare rates for our locality, of which you are always responsible for pre-paying before time of service and no other discounts will be applied. Our self pay rates are often much lower than out of network pricing (if covered).
If you are seen as self pay or out of network you are responsible for 100% of your out of network costs up front and are non-refundable under any circumstance and your payment shall be considered in full and attests you agree to those terms.

Florida Blue has unilaterally restricted access to neurological care, determining that its members—and all BCBS members nationwide—must pay out of pocket or travel further for medically necessary services, intentionally limiting access while increasing costs of care for all.
We understand the burden this creates and strongly encourage you to change insurers immediately to maintain continuity of care and avoid costly out-of-network expenses. We are available to assist you if needed.
