Do you accept my insurance plan? Please refer to our Insurances page to see if your plan is listed. You may also contact one of our offices to find out more.
Do I have a copay? That is determined by your insurance and will vary depending on the insurance company and plan you have chosen. Information may be found on your card or by contacting your insurance company directly. Copays must always be paid at the time of check-in.
Is there a cancellation fee?
A cancellation or no show service fee of $50 may be applied as a result of frequent or last-minute cancellations, no shows, or reschedules. You will always receive a phone call from us 48 hours before your appointment reminding and confirming your appointment with us.
How can I pay my bill or copay? At this time, our offices only accept cash, checks, or money orders. We do not currently accept debit or credit card payments but are looking to bring this convenience in the future. Payments may be made in person or mailed to our Hudson office location.
What do I need to bring with me for my first office visit? You will need your new patient paperwork which can either be downloaded from our website by clicking here, a list of all medications you are currently taking, your driver's license or photo ID, your insurance card, and payment for your copay, if you have one.
I received a bill but do not know why. Please contact our billing company at the number provided on your invoice.
