All requests for medical records must be made in writing via mail, fax, or email with patient properly identified and accompanied by fully-executed release of information.

Beware of text, social media, and email scams! The MIND does not employ text messages nor does it have a presence on any social media platform. Only communication from emails ending in "@malkainstitute.net" are certified, encrypted, official practice email. Any email communication from a domain other than "@malkainstitute.net" is not associated with our practice and should be reported immediately to administration in the Hudson office. Only our EMR provider, PracticeFusion/PatientFusion, or billing platform, Tebra, may send an email/text message on our behalf only to notify you of an upcoming appointment or billing alert.

The MIND will never solicit payment information nor other sensitive information other than what is necessary for scheduling and filing insurance claims and all information shared with The MIND is protected under HIPAA and the HITECH Act.
