Dear patient, on 12/23/25 our practice submitted notice to Humana that, due to their continued breach of contract and illegal and fraudulent downcoding practices, we are terminating our agreement within 90 days of this notice. We have provided their director of provider contracting in Florida and specialist contracting professional ample time to resolve these issues but they refused.
Please contact your insurance agent and change insurers immediately to avoid any disruption in your care.
