Appointment Policy

All patients receive a phone call reminding them of their appointment 48 hours before their scheduled appointment. If we cannot reach you, we will leave a voicemail and try again 24 hours before your scheduled appointment. 


  • Any unconfirmed appointments may be automatically cancelled and charged a $100.00 service fee to allow patients on our waiting list the opportunity to come in sooner. 
  • Cancellations and reschedules made within 24 hours of your appointment are also subject to a service fee. 
  • Should a service fee be charged to you, it must be paid in full before your next scheduled appointment. 


If for any reason you must cancel or reschedule your appointment, please directly notify one of our offices no later than 24 hours before your scheduled appointment. It is the patient's and/or case manager/adjuster's responsibility to directly contact our office for any and all scheduling needs. 

  • A service fee of $100.00 will be charged for all missed appointments or appointments cancelled/rescheduled outside of the 24 hour time window and must be paid in full before rescheduling. 
  • Excessive (3+) cancellations, reschedules, or no shows will place the patient at risk of being discharged from the practice due to noncompliance.

Payment Policy

All co-pays, deductibles, prior balances, and other fees are due at the time of check-in via cash, card, or money order only. 


  • Checks are no longer accepted at any location nor by mail. However, a USPS money order can be accepted in office or via mail as an alternative. Money orders from AMSCOT or elsewhere may be rejected.
  • All card payments are subject to processing fees which affect the amount paid to us
  • Any refund that may be due may be issued only as a credit to the original payment method less the transaction fees incurred on the original transaction. For this reason, any credit on your account will be applied to future visits only. 


If for any reason you are unable to pay, please advise our front office staff and they will be able to assist you. 


For information on prior balances or other billing questions, please call our billing department at 727-862-1135 for assistance.  


Refunds will not be authorized under any circumstances. Should an overpayment exist on your account, it will be offered as a credit to be used in the office for medical services rendered by David W. Malka, M.D., P.A., dba The Malka Institute of Neuroscience and Disease except under extreme and unusual circumstances at the discretion of David W. Malka, M.D., P.A., dba The Malka Institute of Neuroscience and Disease following an extensive account review. Your payment attests that you agree to these terms.

Dishonored Payment Policy

Any and all dishonored payments are subject to applicable fees according to the following schedule: 


  • $25 fee for dishonored payment less than or equal to $50 
  • $30 fee for dishonored payments $50.01-$300
  • $40 fee for dishonored payments $300.01-799.99
  • Dishonored payments greater than $800 subject to fee 5% of the dishonored payment


If payment is not received within 15 days of notice a $40 late fee will also be applied with additional late fees per day for 15 days. Non-payment will result in collection activity and/or legal action for which you are 100% responsible. 


All stop-pays or credit card disputes are unlawful and may be considered fraudulent. As such, they will be reported to the local police and are subject to arbitration of which we reserve the right to prosecute for harassment including false reporting and slander, compensation for damages, time expenditures, legal, and court fees as well your immediate discharge from the practice. 


Prepaid Services

The M.I.N.D. offers various pre-paid services including but not limited to: 


  • Form Completion ($40/page)
  • IME ($850)
  • Miscellaneous time needs for:
    • Phone Conferences ($375 per 15 minutes. Minimum 15 minutes required.)
    • Depositions/Expert Witness ($750 per 60 minutes. Minimum 60 minutes required.)
    • Records Review
  • Narrative Reports ($750/report)


These services vary in price and must be paid in full before the service will be performed. All forms, fees, opinions, and otherwise are non-refundable and non-negotiable. 


Non-payment will result in delay of the service being performed until payment is received in full and stop payments or contested payments will be subject to our dishonored payment policy.


Any reschedule or cancellation will be subject to reschedule/cancellation fee which is determined based on the amount of notice provided to us before the scheduled service is to be performed. 

  • Zero notice/same day: 100% original charge
  • 5-10 day's notice: 50% original charge
  • More than 10 day's notice: $50
